Online appointment and contact information

Your name, email, phone number, selected dentist, preferred appointment date and time window, and message are recorded by Dentaliv to assess your request and contact you. An appointment request is not a confirmed appointment; the clinic will contact you to confirm.

Requests are stored in an administration panel accessible to authorised clinic administrators. The clinic may receive an email notification containing the request number. The notification does not include your medical history or message details.

Do not share medical history, imaging files, identification numbers or payment details through these forms. Patient records and consent documents are not collected through this appointment form.

Reviews

The display name, rating and text you submit are stored for moderation. With your publication consent, approved reviews are publicly visible on this website. Please do not share medical details or other people's personal information. Contact the clinic to request removal. Google reviews are attributed to Google and imported through an authorised business connection.
